New resources for generating healthcare provider referrals

This spring, the American Medical Association (AMA) and the Centers for Disease Control and Prevention (CDC) unveiled the Prevent Diabetes STAT initiative which stands for Screen/Test/Act Today. As a part of that initiative, the AMA and CDC collaboratively developed the Prevent Diabetes STAT toolkit for health care providers.  This toolkit offers providers and practices with a variety of resources to educate and inform patients about prediabetes, and to assist their practices in creating a referral process to link eligible patients to diabetes prevention programs.

In the DTTAC webinar, Sarah describes each component of the toolkit and gives examples of how the components might be utilized in a practice to connect people at risk to the evidence-based lifestyle change program and how health care organizations and/or lifestyle change programs can utilize the toolkit to enhance their participant engagement efforts.
